Geographic and economic influences on benralizumab prescribing for severe asthma in Japan
日本における重症喘息に対するベンラリズマブ処方の地理的・経済的影響
要約
ベンラリズマブは、IL-5受容体を標的とするモノクローナル抗体で、重症の制御不良好酸球性喘息の増悪と経口ステロイドの必要性を軽減できる。日本では、喘息診療に地域差が見られ、処方とアクセスに差異があることが示唆されている。本研究は、全国でのベンラリズマブの地域別処方の変動を定量化することを目的とした。日本の保険請求データベース（NDB、2009年～2019年）を使用して、47都道府県ごとのベンラリズマブの標準化請求比率（SCR）を計算した。他の生物学的製剤のSCR、平均収入などの経済変数、および医師密度との相関関係を一変量解析と多変量回帰分析で評価した。最適な処方を妨げる収入関連の障壁を検討した。
ベンラリズマブのSCRは、都道府県ごとに40.1から184.2まで大きく変動した。SCRはオマリズマブ（r = 0.61, p < 0.00001）およびメポリズマブ（r = 0.43, p = 0.0024）と強い相関を示しました。平均月収もベンラリズマブのSCRと正の相関があった（r = 0.45, p = 0.0016）が、ライフスタイル要因は有意ではなかった。呼吸器専門医の密度もSCRとわずかに相関していた（r = 0.29, p = 0.047）。多変量回帰分析では、平均収入が最も強力な予測因子であることが示された（B = 0.74, p = 0.022）。ベンラリズマブのSCRは、医療インフラや人口要因よりも収入指標と強く関連していた。多くの地域で低いSCRが見られ、明らかな処方ギャップが存在していた。日本の所得層間で高度な喘息治療へのアクセス障壁が依然として不平等であることが明らかになった。専門医の配置とともに、価格の手頃さに対処することで、より良い処方の質と喘息の結果を達成することが可能となるであろう。
